If you wanna apply to Ectoplasm Factory [DX], please use this template as your application form.

1. What is your main character's name?

2. What is your time zone?

3. What other guilds have you been in prior to applying?

4. What is your current guild and what are you reasons for leaving? (If currently guild-less, why did you leave your last guild?)

5. What are your best times and average times in Underworld? List your each area.
